At the time of your visit. . .

We ask patients to arrive a few minutes before their scheduled appointment time to fill out a few forms. We ask you to fill out the patient registration/information form , which you can print by clicking on this link. or we will provide you with one when you check in.  This provides your perinatologist with some basic information regarding, you, your medical condition, family medical history, and the availability of previous medical tests.  In addition, when you arrive, before we can evaluate and treat you, we will ask you to review and sign a Consent to Treat form and a standard HIPPA form which outlines our policies and your rights concerning your medical information.

If you would like to click on the link above, print, and complete these forms and bring them to you to your appointment, it will save time during your visit. Otherwise, you will asked to complete these forms before you are seen.

You will be greeted when you arrive at the Center. One of our office staff will ask for your insurance card and any co-payment that may be due at the time of service. Many, but not all,  patients are required by their insurance company to pay a copay. We honor most major insurance carriers and most procedures are covered. The Department of Obstetrics and Gynecology at University of Colorado provides specialty care for the major HMO, PPO, Medicaid, CHP, and commercial insurance plans in Colorado and the Rocky Mountain Region.

If you do not have insurance, we will ask you to provide some payment at the time of service.  And, you may be asked to sign some additional paperwork.  Our patient representatives will help you with payment options.

We strive very hard to remain on schedule. However occasionally and especially in the late morning and afternoon, we can run a few minutes behind when unexpected medical problems arise that require immediate attention.

Please click here to view our fees for the most common clinical services we offer. Please note that we accept most insurances and have contracts that discount these fees for those payers. We also offer self-pay and other discounts for patients that qualify.
